Home Social Worker

General Description:
This position is responsible for enhancing families’ capacity to support their own growth and
development by receiving referrals, helping clients with coping skills and providing long term support
services to mentally or physically challenged band members. This position also assessing the clients
needs and makes referrals for other supportive social services.
Responsibilities:
• Interviews clients and families and completes home assessments to help determine service
needs (e.g. parenting, mental health addictions, mom self-care, general self-care)
• Provides counseling and therapy support to resolve social and personal problems
• Provides clients with information and education to assist with their skill building needs and
other support systems
• Receives intakes and investigates adult protection concerns.
• Provides referrals to other support services
• Acts as client advocate and prepares letters of support of professional services
• Develops/advises on social policy issues
• Provides administrative supervision to the workers in Assisted living and home care programs
• Performs other related duties as required by Supervisor such as producing reports, attending
meetings, providing information sessions, etc.
Qualifications:
• Post-secondary education in social work, either a bachelor’s degree in social work, and at a
minimum three years’ experience in strength-based social work practice in community or
government setting.
• Must be eligible to be registered with the Nova Scotia Association of Social Workers.
• Must be willing to work occasional flexible hours to perform work in the client’s home and
community.
• Flexible and responsive to deal with changing needs of client circumstances.
• Strong interpersonal communication skills with ability to document client case notes and
provide accurate, non-judgmental assessments.
• Ability to work independently with minimal supervision within appropriate professional
boundaries.
• Proficient in using Microsoft Office Suite.
• Possess cultural awareness and sensitivity
• Must have access to a reliable vehicle and valid Nova Scotia Driver’s License
• A criminal records check and vulnerable sector check is required. Having a criminal record is
not necessarily a determinate factor.

Working Conditions
• Normal work week is 35 hours/week, 7 hours/day with need to work some flexible hours
• This position will be required to travel
